What is a DMV Reinstatement Form?
The DMV Reinstatement form (DR 2870) forms are to assist you in getting your restricted license reinstated after it has been revoked for a DUI. This reinstatement form is something to be filled out after you have finished your period of revocation as well as all other requirements from the court and the DMV.
The DMV will not issue a reinstatement of your license if you have not completed all requirements including all fines and costs. A reinstatement fee of 95 dollars must be paid before you will be reinstated. Level II alcohol treatment and therapy will need to be completed. You will have to get an interlock installed in your vehicle and any other vehicle that is registered in your name.
There are mandatory restrictions that are associated with reinstatement. These are non-negotiable. A certification of your knowledge of alcohol and driving will be requested of you. If you live out of state you will have to provide the DMV will all of that information as well. This form also outlines all of the financial responsibilities of reinstatement, including special, SR-22 auto insurance.
